Admission Procedures and Application Form
NOTE: This Application Form can also be used as a guide to the Admissions process. It links you to the web pages you will need to complete the Form.
Do not complete or send this form until you have reviewed the Degree Declaration information page and the Admission requirements Section 1-5 of the Procedures page. These pages address
- your previous degree or obtaining its equivalent,
- declaring your Degree or Certification intentions,
- required reading of the overview and website materials,
- calling Dr. Cohen, and
- determining how much scholarship funding you will need, if any.
The information on the above pages and their links prepare you to answer the questions that appear on this Form.

NOTE: To avoid the consequences of eroding trust while you participate in the program please do not misrepresent this figure. Please keep within the purpose and spirit of the Scholarship Program.
If you decide not to complete your program, IGE will refund you a minimum of $1200.00. from what you have paid directly in tuitions above $2000. You may also permanently switch into the Certificate Program at that time.

NOTE FOR DEGREE WAIVER APPLICANTS
Bachelor's Degree Waiver Applicants:
You must successfully complete the Orientation course and your equivalency degree waiver process before full acceptance to the Degree Program. You may proceed as a student and upon completing the Orientation Course you may work on your degree waiver and other courses. Its cost varys and is not covered by the CCP Tuition plan. A Bachelors Degree is not needed for CertificationMasters Degree Waiver Applicants:
You must successfully complete the Orientation course and your equivalency degree waiver before full acceptance to the Doctoral Degree Program. You may apply and take courses with the understanding that you will complete your MS equivalency documentation by the end of your first semester's courses.

We have found, over the years, that asking people to fill in a series of blanks or boxes on a prepared form is rarely the best way for them to tell us about themselves and why
they are interested in earning a Bachelors, Masters, or Doctorate. We have, as a result, left things quite open-ended and invite you to communicate with us in a manner that makes the most sense to you yet allows us to learn as much about you as possible so we may be sure this program will meet your goals. However, your success and comfort with the described program procedure is the best way for you and us to make that determination.

SECTION EIGHT
APPLICATION REFERENCES
Keep a hard copy and email copy of the application form for your own records.
Along with this application we will need:
____ A letter from you which provides us with information on your proposed field of study and what (if anything) you have already done in this field academically, professionally, as a hobby, a volunteer, etc., and____ If you are applying for a Masters or Doctorate, and already have a Masters thesis or a Doctoral dissertation topic in mind, or if you have already done some work on such a project, you should describe this in some detail, enclosing additional materials as necessary and appropriate.
____ A copy of all transcripts and other academic records as specified on the application. Photocopies of transcripts are acceptable for submission with the application.
____ At least 2 letters of reference from people who know you well. If possible, at least one should be from an employer or person familiar with your career and one should be from a person familiar with your academic performance.
