Cohen, Director ....360-378-6313 ....Email   Student Coop and Course Enrollment Contract To be eligible to enroll in the Cooperative, you must have completed the Orientation Course and Program Application The following contract is made to help IGE-IUPS applicants fulfill their attractions to the purpose of the IGE Graduate Student Cooperative for the benefit of all concerned. Date: ____________________ Through this contract, I, ___(your name)_______, commit to myself and the Graduate Student Cooperative that I will participate in the Cooperative to the best of my ability and will start my next semester and courses on DATE____________ and complete that semester on DATE ____________. My first semester tuition payment of $1500.00 accompanies this enrollment contract and has been sent along with a hard copy of this Contract to Karla Gulke, IGE Cooperative, 20887 West Bulsen Road Mt. Vernon, WA 98274 I will email a copy of this form to igecoop@ecopsych.com   Coursework I understand that IGE Cooperative degree program semesters are 6 months long and officially start in January, April, July, and October. I am aware that to sustain my membership in the Cooperative a payment of no less than $1500 is made to start my first semester. Upon the start of each following semester a payment of $1000, accompanied by a payment form, is due until my total tuition contribution is complete. I may take 6-15 credits each semester and I can petition the support group coordinator for special permission to take 18 credits I can start my course work before the semester begins when classes are available and may complete them, when necessary, during the following semester. I will make an original proposed program schedule of the time I will take the courses and have it approved by my IGE support group and mentor.___________ I will inform my support group and others involved, about making this commitment and my progress with it ________   Cooperative learning and administration Mentoring: I will enlist the assistance of my guide group when needed, and offer assistance to its members when possible. Whenever I make a payment to the Cooperative it will be accompanied with a Payment Form I acknowledge that this contract applies to all areas described on the IGE Cooperative Requirements page. I acknowledge that to be a member in good standing of IGE-IUPS Student Co-op I must live up to the commitments I have made here, to additions to this contract as they arise, and that they are all part of the IGE-IUPS degree program for which I can get field experience credit hours.
